Submit the online form using the button at the bottom.

Send your biopsy specimen using the pre-paid mailer and write “Online Submission” on the mail-in form.

How to Create a Desktop Shortcut to the Online Form (Windows)
1.
Open the online requisition form.
2.
Copy the URL (highlight it and press “Ctrl” + “C”)
3.
Right-click on your desktop and select “New” > “Shortcut.”
4.
Paste the URL (“Ctrl” + ”V”) and click “Next.”
5.
Name the shortcut (e.g., “SOPS OPR”) and finish the setup.
